Face

Blepharoplasty [Upper/Lower]

This procedure is performed to remove excess skin and fat of the upper or lower eyelid associated with aging. The upper eyelid procedure is done for functional problems like limiting your field of vision or to enhance the shape and contour of your eyes. As for the lower eyelid, sometimes called “eye bags”, it is associated with looking tired and also with premature aging.Functional upper eyelid surgery may be covered by your insurance plan if it causes your visual field to be blocked. Dr.Kahng's office will help facilitate getting an appointment with an opthalmologist. To rejuvenate the whole eye region, evaluation of the eyebrow/forehead position as well as the lower eyelid needs to be done by Dr.Kahng.

Procedure time and Recovery Blepharoplasty

Dr.Kahng does all his eyelid surgeries in his AAAASF certified outpatient surgery center within his office. Surgery typically takes 1-2 hours and is done under intra venous sedation for patient’s utmost comfort and safety. Patient need to be NPO for 8 hours and need to arrive at the surgery center 1 hour prior and usually stay in the recovery room for an hour prior to being discharged. Sutures are removed in 5-7 days and patients are restricted from any strenuous physical activity for 1-2 weeks.

Rhinoplasty

Reshaping one’s nose is one of the most challenging and sophisticated procedures in plastic surgery. Like any other procedures, minor changes to major changes to the appearance of the nose can be accomplished. However, even minor changes result in dramatic changes of the facial impression and appearance. Common reshaping is done to remove the dorsal hump, increasing or decreasing the width, increasing/decreasing the size of the nasal tip and height of the tip, and changing the nostril widths. Asian and non Asian rhinoplasty differ in anatomy and procedure type. Most Asian rhinoplaty is done via an open technique procedure and non Asian rhinoplasty can be via a closed technique procedure. Also it is important to tell Dr Kahng about any breathing problems before any nose procedures. If on examination there is nasal obstruction, reconstructive procedures can be done along with reshaping the nose. This portion can be covered by insurance depending on the type and plan of the insurance you have. Like all plastic surgeries, Dr Kahng will individualize the nose procedure to achieve each patient’s desired outcome. The final result of the nose can sometimes take up to 12 months due to prolonged swelling after the surgery.

Chin Augmentation [Geneoplasty]

This procedure can be a standalone procedure or combined with other facial procedures like rhinoplasty to bring balance to the face. Different types of silicone implants are used to reshape the structure of the chin and make the appearance more masculine or feminine. Incision approaches are chosen depending on individual needs and the outcome of the appearance. Dr Kahng will give individual recommendations of the type of implant and approach based on your facial structure.

Lip Augmentation

Perma lip(FDA approved) augmentation is an option for patients who desire to have a permanent enhancement of the upper/lower lips. The procedure can be done under local anesthesia. Custom soft medical grade silicone implants are used to enhance the lip volume. The implants are soft and pliable, indistinguishable from one’s on soft tissue. Perma lip provides a natural look and feel.

Endotine Browlift

The appearance and the position of the eyebrows change with the aging process. Men typically have the brow positioned at the bony part of the upper eye socket and women typically have the brow position at about 1 cm above the bony part of the eye socket. Brow lift is a surgical procedure to address the descent or downward migration of the eyebrows as one ages. This is commonly seen in the late 30s and early 40s. Youthful appearance diminishes and patients may comment on looking tired and altering the facial expressions. Dr Kahng uses the latest technique with minimally invasive endoscopic approach. This will lift and smooth your forehead. Also, if patients desire to also permanently remove the wrinkles between the brows, the muscle can be treated while doing the endoscopic brow lift.

Face/neck lift

A facelift or rhytidectomy surgery helps to rejuvenate the facial appearance by tightening the muscles(SMAS and platysma) in the face and neck, while removing and repositioning excess skin, fat and deep tissues. Dr Kahng further restores your youthful look by adding fat grafting to give a natural 3-D rejuvenation of your facial features. A facelift expertly performed will make you look your best and reverse some of the effects of gravity and aging. You will see immediately improvements in your chin, jawline and cheeks. Other facial procedures such as blepharoplasty, rhinoplasty, brow lift and liposuction can be performed safely while performing a facelift. Do to the extensive detailed nature of the procedure and for patient safety, it is impossible to perform this type of a procedure under local anesthesia only. It is recommended that an anesthesiologist perform either a deep intra venous sedation or general anesthesia. Dr Kahng also offers surgical treatment to permanently treat crow’s feet. Good candidates are patients who used Botox to temporarily treat crow’s feet but want permanent results.

Neck lift

Isolated neck lift only can be performed without a face lift. This is done to improve the central portion of the neck under the chin but does not improve the jawline or jowling.

Midface lift

This is a minimally invasive surgery that corrects the region of the midface. With aging, lines appear in the cheek area called the naso jugal fold and prominence of fatty tissue can appear above the naso labial fold. Surgical correction can be done with multiple modalities such as using the midface endotine device, or using a combination of fat grafting and Accuscult to contour the malar region like using laser lypolysis to treat the bulging area over the NLF.

Stem Cell/Fat grafting

This procedure is done alone or combined with other facial procedures. With aging, certain compartments of the face (upper eyelid, tear trough, temporal, malar, cheek, lips) loses volume. To re establish youthfulness, fat is used along with its stem cell to correct any volume deficiencies.

Breast

Breast Augmentation

Breast augmentation is one of the most common procedures in plastic surgery and most common surgery performed by Dr Kahng. An implant is placed beneath the native breast tissue to enhance the volume and shape of the breast. There are multiple surgical approaches/incisions (peri- areola, infra mammary fold, axilla) and type of implants ( saline vs silicone cohesive gel vs anatomical form stable silicone gel). Dr Kahng believes in individualizing and customizing your breast augmentation procedure when you have a complimentary consultation. Dr Kahng prefers the co gel (cohesive gel)/gummy bear implants to achieve a softer more natural look. The silicone cohesive gel/gummy bear have been FDA approved since 2006.

Asian Breast Augmentation

Mastopexy [Breast Lift]

Candidates for this breast procedure are those who have adequate breast tissue but with age, the breast tissues have sagged with gravity and also after having children. Another cause is also due to medical or surgical weight loss. Breast lift is performed to elevated the nipple when it is at the level of the infra mammary fold or below. Patients may have adequate volume of breast tissue or have lost the volume. Depending on the amount of excess skin and volume of breast tissue, patient will require one of the three types of breast lift (donut lift, lollipop lift, anchor lift) with or without an implant.

Breast Cancer Reconstruction

Breast reconstruction should be an integral part in the discussion of treatment and plan of breast cancer patient who is undergoing a mastectomy. Patients should have a consultation by a plastic and reconstructive surgeon prior the oncologic surgery to discuss the different types of reconstructive surgery and to individualize not only the oncologic treatment but also the timing of the reconstructive procedure.

The method of breast reconstruction is divided into two types: autologous (one’s own tissue such as muscle/fat/skin) vs. tissue expander/implant reconstruction. Autologous breast reconstruction is done with transfer of tissue with its blood supply like pedicled TRAM flap or disconnecting the blood supply and reconnecting it at a different site (micro vascular surgery) like free DIEP flap. Each has its own risks and benefits. Tissue expander reconstruction is less tasking on the body and allows a quicker recovery. However, requires multiple small procedures. A “balloon” like device is placed to either stretch the skin or maintain the skin until a permanent silicone cohesive gel implant can be placed. Both of these reconstructive procedures at the end of the treatment involve reconstructing the nipple and areola.

Dr Kahng is also one of the few reconstructive plastic surgeons who offer a one stage implant reconstruction to patients with certain types of breast cancer that do not require radiation. Also this is ideal for patients who have a strong family history or who test positive for the breast cancer gene undergoing prophylactic nipple sparing mastectomy to prevent breast cancer. Dr Kahng uses a permanent cohesive silicone gel implant along with an acelluar dermal matrix (Alloderm/Strattice) sling.

Reduction Mammaplasty [Breast Reduction]

Breast reduction procedure is performed hen patients have more than adequate breast tissue that cause symptoms of back/neck/shoulder pain, as well as difficulty maintaining personal hygiene and developing rash under the breasts. Patients usually experience difficulty finding adequately fitting bras and state the large breasts interfere with daily physical activities. Breast reduction is very effective in relieving such symptoms. This surgery can be covered by your insurance depending on your plan. Breast reduction not only reduces the volume but also lifts the breast tissue.

Male breast Reduction

Gynecomasty is a condition of over-developed breast tissue and tissue in men. It is commonly associated with aging and male hormonal changes, both young and old due to hormonal imbalances. Now a day, it is also commonly seen after significant medical or surgical weight loss procedures in men. With gynecomastia, the condition not only is seen physically but can affect the patient’s mental state, causing social withdrawal. Depending on the type and amount of different tissues, male breast reduction is done with liposuction and removal of glandular tissue. However, if there is excess skin, either a lollipop or inverted T breast lift maybe need as seen in weight loss patients.

Body

Tummy Tuck [Abdominoplasty]

This procedure is performed to contour the abdominal region-muslce, fat, skin. Candidates for this procedure are usually women due to post pregnancy tissue changes. It is also done for both men and women after massive weight loss. The abdominal wall and soft tissue under goes irreversible changes causing the rectus abdominus muscle ( “6 pack” muscle) to separate and have excess loose skin and fatty tissue. Depending on the extent of the soft tissue changes, patient’s maybe candidates for mini-tummy tuck to full tummy tuck. This procedure may be combined with liposuction to contour the love handle areas or the pubic areas. For certain candidates, Dr Kahng will perform a drainless abdominoplasty to further provide comfort during the recovery.

Mommy Make Over

After pregnancy, woman’s body goes through both hormonal and structural changes to different parts of the body. The significant changes are seen in the breast, abdominal areas, and vaginal areas. The breast is reshaped by augmentation with either saline or silicone gel implants along with or without a breast lift and the abdomen is reshaped with an abdominoplasty and liposuction procedure.

Labioplasty and Vaginal Wall Reconstruction

Due to the process of child birthing and hormonal changes, the external and internal anatomy of the vaginal area is changed. The external genitalia (labia minora) may acquire excess skin, causing discomfort and the birth canal can also be stretched and cause urine leakage. Sometime labial enlargement is congenital. Labioplasty to reduce the size of the labia minora and vaginal reconstruction is necessary to restore the anatomy and re suspend the bladder. These procedures can be covered by your insurance depending on the plan.

Liposuction

This procedure is minimally invasive and is another powerful tool to contour the face or body. It is to reduce localized fatty tissue. Common areas of treatment include the abdomen, back, thighs, upper arm, neck or face. Dr Kahng uses the Power Assisted Liposuction System. Dr Kahng uses the latest advanced technology with laser assisted lipoplasty (Accuscultp) to both liquefy and to tighten the skin of the face and body. Using this FDA approved technology, post operative bruising and swelling is minimized. Dr Kahng also uses this technology to correct contour irregularities for patients who have the “lumps and bumps” after liposuction in the past. Post operative garments are vital in contouring the face or body and Dr Kahng recommends all his patients to wear the compressive garment for a period of 3-6 months to optimize the results.

Brazilian Butt Lift

This procedure is to lift, enhance, and volumize the buttock with the patient’s own fatty tissue in conjunction with contouring the surrounding areas of the trunk (abdomen, back, flanks) and thighs (outer, inner, banana roll. Traditional mechanical liposuction with atraumatic cannulas is perform to harvest and collect the fat which is then carefully transferred to the buttock area. If patients do not have enough fat that can be harvested, the only surgical option is to use specialized silicone implants. Dr Kahng recommends patients to always be in a prone position for 2 weeks and to wear a special compressive garment to mold and shape the buttock region for 3-6 months. When patients do not have enough fat to perform the fat transfer, Dr Kahng will recommend gluteal silicone implant surgery.

Gluteal Implants / Augmentation

Unlike in the past, gluteal augmentation with silicone implants has been safe when performed properly. The risk of infection is low comparable to a breast augmentation. The silicone implant different than a breast implant. The implant is placed through a well hidden incision in the gluteal crease and under the muscle. Patients can achieve a natural or fuller look that is permanent. The implants do not have to be changed over time. The recovery however is long and patients need to follow a strict post operative care instructions to minimize any complications. Dr Kahng will provide a detailed recovery course for each individual patients.

Body Lifts

Patients who have lost a large amount of body weight, either by diet or by surgeries like lap band or gastric sleeve, have excess skin in different parts of the body like abdomen, back, buttock, arms, thigh, face, and neck. Contouring the body from head to toe requires multiple procedures and some of the procedures can be covered by your insurance. All of these procedures can be done safely as an outpatient procedure.

  • Belt Lipectomy-removal of skin and fat from the lower part of the abdomen and lower back/buttock junction.
  • Panniculectomy-removal of skin and fat in the lower abdominal area only. However, to achieve an ideal reshaping of the anterior abdominal wall, an abdominoplasy is recommended where not only excess skin and fat is removed but also the abdominal wall muscle is sinched/plicated.
  • Brachioplasty-removal of skin and fat from the upper arm. Dr. Kahng uses a technique to minimize complications compared to the traditional brachioplasty where skin flaps are elevated. Dr. Kahng combines the use of power assisted liposuction (PALS) and skin removal for a quicker recovery.
  • Bra line lift-removal of skin and soft tissue in the mid portion of the back where the incision line is hidden in the bra line.
  • Inner thigh/knee lift-removal of skin and fat from the inner part of the knee and fat. Dr. Kahng uses a technique to minimize complications compared to the traditional inner/medial thigh lift where skin flaps are elevated. Dr. Kahng combines the use of power assisted liposuction and laser lipoplasty along with skin removal for a quicker recovery.
  • Mastopexy/breast reduction-removal of skin, fat and breast tissue.
  • Face/neck lift-removal of excess skin and tightening facial and neck muscles.

Hand Surgery

Dr Kahng provides an extensive comprehensive care for patients with hand problems. Procedures performed by Dr Kahng are as follows: simple laceration repairs to the fingers/hand, repair of complex tendon lacerations, carpel tunnel release, trigger finger release, treatments for De Quervain’s tenosynovitis, and treatments for Dupytrene’s contracture. Hand/occupational therapists are integral part of hand rehabilitation to achieve a normal function and Dr Kahng will arrange the therapy with your insurance carriers.

Scar Reconstruction

Scar management is a vital component of plastic surgery because of the significant variation in scarring. Patient’s ethnic background can predispose them to bad scarring where it is from elective or non elective surgery. Scars can cause functional problems in particular over joints causing limitation in the range of motion. Dr Kahng will provide many options in scar management from noninvasive to invasive procedures. Scars take up to 1 year to mature and remodel and in that process, silicone tape therapy and intra lesional steroid injection can be utilized. Surgical scar excisions/revisions can be done if refractory to initial treatment and especially if limitation of range of motion of a joint.
